| 연구요약 |
국문 마취 관련 사망률은 주술기 환자 안전에 관한 기본적인 객관적 지표입니다. 다양한 환자감시장치 및 마취 약제의 개발/적용은 과거에 비해 마취 관련 사망률이 크게 줄어들고 있는 것으로 알려져 있으나, 국내에서는 이에 관한 연구가 외국에 비해 상대적으로 부족한 형편입니다. 최근 국내에서도 다양한 임상분야에서 수술보다 보다 비침습적인 치료적 시술과 환자의 불편감을 경감시키기 위한 진단적 시술들이 수술실 외부 공간에서 마취통증의학과 전문의가 제공하는 마취서비스를 통해 이루어지고 있으나, 이에 대한 안전성에 대한 연구는 전무합니다. 이에 연구자들은 최근 4년 11개월간 본 병원의 수술실 외부공간과 수술실 내에서 이루어진 시술/수술 마취와 연관된 사망률을 비교 분석하고, 특히 수술 후 48시간 이내 사망한 건들에 대한 의무기록 검토를 통해 사망의 주요 원인을 ‘환자의 질환 및 상태/수술 관련/마취 관련/기타’로 분류하여 각기 구체적인 원인기전에 대해 탐색하고자 합니다. |

영문 Perioperative mortality is an objective measure for quality of care and patient safety in the perioperative period. It is clear that anesthesia-related mortality rates have declined over the past two decades as a result of improvements in monitoring, medications, and patient care. Over the last decade, there has been a global growth of interventional procedures being performed outside of the traditional operating room locations. In addition, more and more technically demanding procedures are performed outside the operating room in more and more sick patients. Similar to other developed countries, this trend produces an increasing demand for non-operating room anesthesia (NORA) in South Korea. However, there are few studies on the relative safety of NORA compared with operating room anesthesia (ORA). Especially, although many practitioners suggest that the volume of NORA cases in our country is increasing, it is not clear precisely how frequently anesthesiologists practice outside operating room because of no domestic study describing the features of NORA. Therefore, the aim of this study was to characterize NORA cases and compare the characteristics, and the rates and causes of deaths occurring within 48 h of surgery (48-h mortality) of NORA cases with those of ORA cases in surgical population over an 59-month period at a single tertiary university hospital. |
