1) 연구 배경
수술중 안정적인 혈압을 유지하는 것은 매우 중요한 일입니다. 수술중 혈압과 수술 후 예후와의 밀접한 상관 관계는 이미 널리 알려져 있습니다. 혈압은 심박출량과 (CO) 전신 혈관 저항 (SVR)의 곱으로 표현 할수 있으므로, 안정적인 혈압 유지를 위해서는 이러한 2가지 구성 요소를 각각 이해하고 추정할 필요가 있습니다. 하지만 불행이도, 전신 혈관 저항은 아직까지 수술중 직접 측정할수 없습니다. 다만, 심박출량, 중심정맥압 (CVP), 평균 동맥압 (mean ABP)을 통해 간접적으로 추정할 수 있습니다 (SVR = [mean arterial pressure – CVP] / CO). 원칙적으로, 전신혈관 저항을 정확히 추정하기 위해서는 위의 3개지 변수에 대한 정보가 모두 필요합니다. 하지만 임상적 범위의 중심 정맥압 (5~15 mmHg)은 평균 동맥압 (60~120 mmHg)에 비하여 상대적으로 작은 수치이므로, 과연 중심정맥압이 전신혈관 추정에 얼마나 중요한지에 대해서는 의문이 있을수 있습니다. 
 

2) 연구 가설 및 목적
고정값을 이용한 전신혈관저항 추정은 정확한 중심정맥압 측정을 이용한 추정을 대체할 수 있을 것이다. 

3) 연구대상자의 선정/제외 기준, 목표한 대상자 수 및 산출 근거
a. 선정기준
심장 수술 또는 신경외과 수술을 시행받는 환자로 수술 중 중심정맥압 및 심박출량을 측정한 환자. 	
b. 제외기준
- 데이터 오류 또는 손실로 인해 분석에 제한이 있는 경우
- 취득된 데이터중 다음에 해당하는 구간은 분석에서 제외
- NA values for CO, CVP, mean ABP
- Mean ABP>140 or < 30 mmHg
- CVP >30 or <0 mmHg

c. 목표한 대상자 수 및 산출 근거
* 본원 (충남대학교 병원)에서 2021년 2월부터 3월까지 수술을 시행 받은 환자 중 위 선정 기준에 해당하는 환자. 
* 대략 40명가량 해당

   
4) 연구방법
 a. 전반적 개요
    * 후향적 관찰 연구
    * 수술 중 환자의 생체신호 data를 vital recorder를 통해 수집
    * 다음의 3가지 종류의 CVP를 이용하여 SVR 추정
    - Fixed CVP: 0, 5, 10, 15, 20 mmHg 각각으로 고정된 CVP
    > SVRf0, SVRf5, SVRf10, SVRf15, SVRf20 
    - Random CVP: 1~20 random sampling으로 형성된 CVP
    > SVRr
    - Actual CVP: 실제로 측정된 CVP
    > SVRa
    
* SVRa 와 SVRf0, SVRf5, SVRf10, SVRf15, SVRf20, SVRa 비교

1) background
Maintaining stable blood pressure of patient is a core role of anesthesiologist along with providing adequate anesthesia. There are abundant evidences showing clinical outcomes closely related to the intraoperative management of hemodynamics. Since the blood pressure is a product of cardiac output (CO) and systemic vascular resistance (SVR), it is also essential to understand and estimate each of these components. Unfortunately, however, there are still no available tool that can directly measure SVR intraoperatively. It can only be indirectly estimated using mean arterial pressure, CO, and central venous pressure (CVP; SVR = [mean arterial pressure – CVP] / CO). Principally, therefore, a clinician has to know all these three variables to estimate systemic vascular resistance correctly.
 However, since the clinical range of CVP (typically 5 to 15 mmHg) is far below the range of mean arterial pressure (60 to 120 mmHg), it is questionable that the impact of CVP on the estimation of SVR really matters. There are few studies specifically answered to this question. The accuracy and feasibility of using an arbitrary value rather than actual CVP for the estimation of SVR is not known, especially in the scale of intraoperative period. We, therefore, calculated the SVR using randomly changing CVP (1 to 20 mmHg) and a fixed CVP (0, 5, 10, 15, and 20 mmHg) then compared them with the actual SVR (by measured CVP) using vital records acquired from neurosurgical and cardiac surgical patients. 

2) Primary hypothesis 
A fixed value may be feasible for the estimation of the SVR

3) Sample size and the Inclusion/ exclusion criteria
a. Inclusion
patients who underwent neurosurgery or thoracic surgery with invasive hemodynamic monitoring consist of central venous pressure and cardiac output during two months of period (February to March, 2021)
b. Exclusion
- Missing or error data which precludes the analysis
- NA (not available) values for CO, CVP, mean ABP
- Mean ABP>140 or < 30 mmHg
- CVP >30 or <0 mmHg

c. sample size
According to the aforementioned criteria, about 40 patients are expected to be included
