| 연구요약 |
국문 최근 조영 증강 내시경(Image-enhanced endoscopy, IEE)이 발전하여 그 유용성이 증가되고 있다. IEE 중에서는 Olympus의 narrow band image(NBI)가 대표적이며, NBI를 이용한 확대 내시경이 조기 위암의 수평 경계 평가에 있어 기존의 색소 내시경보다 더 정확하거나 비열등성을 가진다는 보고가 있으나 관련 연구나 데이터는 아직 부족한 상태이다. Pentax의 optical enhancement(OE)는 mode-1, mode-2 으로 두 가지 버전이 있으며, 광학 필터를 사용하여 장관 점막의 미세 표면 구조와 미세 혈관 패턴을 관찰할 수 있게 한다. OE(mode-1)가 조기 위암의 수평 경계 평가에 있어 유용하다는 보고가 있으나 NBI에 비해 상대적으로 데이터가 더 부족한 실정이다. 우리는 조기 위암과 고등급 이형성 선종의 내시경적 점막하 박리술 전에 종양의 경계를 색소 내시경을 이용한 백색광 내시경과, OE(mode-1)을 이용한 조영 증강 내시경으로 각각 평가할 것이다. 이를 통해 산출한 결과에 따라 조기 위암의 수평 경계 평가에 있어 OE를 이용한 조영 증강 내시경이 색소 내시경보다 더 정확한지 여부를 확인할 것이다. |

영문 Recently, image-enhanced endoscopy (IEE) has been developed and its usefulness has increased. Among the IEEs, the narrow band image (NBI) of Olympus is representative, and a magnifying endoscope using the NBI is more accurate or noninferior than the conventional chromoscope in evaluating the horizontal boundary of early gastric cancer. It is reported that relevant research and data are still lacking. There are two versions of optical enhancement (OE) of Pentax in mode-1 and mode-2, and it is possible to observe the micro surface structure and microvascular pattern of the mucous membrane using an optical filter. Although (OE mode-1) has been reported to be useful in the evaluation of horizontal boundaries in early gastric cancer, it is a fact that data are relatively lacking compared to NBI. We used white light endoscopy using chromoendoscopy and OE (mode-1) for the border of the tumor before endoscopic submucosal dissection of early gastric cancer and high grade dysplastic adenoma. Each is evaluated by contrast enhancement endoscopy. Thereby, based on the calculated result, it is confirmed whether the contrast enhancement endoscope using the OE is more correct than the dye endoscope in the horizontal boundary evaluation of early gastric cancer. |
